Primary care provider

Your primary care provider remains an important member of your cancer care team during your cancer journey. Your primary care provider is your family doctor or nurse practitioner. Please contact your primary care provider:

  • To renew all drugs prescribed by your primary care provider.
  • If you have changes in your health between visits to the hospital, like if you have a cold or the flu.
  • For health problems your primary care provider has been treating you for, like a heart condition, high blood pressure or diabetes.
  • To help manage your symptoms, like pain, that may be related to your cancer.

Your primary care provider can call the hospital for information about your care or to tell us about changes in your health at any time. Your cancer care team will communicate with your primary care provider as needed about your cancer and your cancer treatment. After your cancer treatments are over, your cancer care team will partner with your primary care provider to make sure you get appropriate care and follow up.
